EXPEDISE IN-HOUSE FINANCIAL ASSISTANCE FOR WORKFORCE CUSTOMERS – POLICY AND IMPLEMENTATION GUIDELINES

Policy Statement
Expedise PH is committed to protecting the health, safety, and financial well-being of all workforce customers deployed to clients. In support of this commitment and in compliance with DOLE Department Order No. 174, the company provides an In-House Financial Assistance Program that offers coverage for accidents, hospitalization, disability, and death.
This program is fully funded by Expedise PH and is administered directly by the Customer Care Team under the Workforce Division.

Definition of Terms
- Accident – A sudden and unexpected event causing injury, not due to illness or intention.
- Beneficiary – The person officially named by the customer to receive Financial Assistance benefits in case of death.
- Claim – A formal request for benefit payment filed by or on behalf of the employee.
- Coverage – The list of benefits provided under this Financial Assistance policy.
- Exclusion – A condition, situation, or event that is not covered by this Financial Assistance policy.
- Employee or Customer – Shall mean any personnel of the Company that has established being employed for whatever employment nature (probationary, fixed-term and/or regular)
- Fixed-Term – A workforce customer has a job with a clear start and end date, usually tied to a project or task.
- Hospitalization – Admission into a licensed hospital that requires at least one overnight stay.
- Probationary – A workforce customer under evaluation, typically during the first 6 months of employment.
Scope And Eligibility
The policy shall apply to all workforce employees who meet the following conditions:
- All Customers of the Company stated herein.
- Assignment: Actively deployed to a client site through Expedise PH
- Enrollment: Automatic upon first day of deployment
All employees must provide their respective beneficiary information upon onboarding with Expedise PH. Which may be updated by employees should there be changes.
Financial Assistance Coverage

| Accidental Death while off Duty | ₱20,000 payable to named beneficiary |
| Permanent Disability | Up to ₱300,000 based on severity |
| Murder and Assault | ₱300,000 in the event of death due to violence |
| Medical Reimbursement | Up to ₱30,000 for accident-related medical care |
| Burial Assistance | ₱20,000 for funeral-related expenses |
| Hospital Income Support | ₱180/day (max 30 days) for hospitalization due to accident |
Note: These benefits are subject to review and adjustment based on company policies and capacity.
Claims Filing Procedure
Reporting
Customers must report any incident within 24 hours to their assigned Customer Care Representative with the help of Customer Concierge. Should an incident occur on a long weekend, holiday or any other days with the same nature, Customers must ensure to file their report the next business day.
Requirements
Upon submission of incident report to Customer Care, Customers shall be requested to fill-up the online claims form and submit the following documents within 5 working days of the incident:
- Valid government-issued ID (with 3 specimen signatures)
- Photo of injury (for accidents or dismemberment)
- Original official receipts for medical expenses
- Original or Certified True Copy of the Following: Incident Report or affidavit, Medical abstract or death certificate, Police Report and/or other certifications from BFP or other government agencies (if applicable)
Customer Care representative shall validate filed claims and shall evaluate possible claim amounts and notify the Finance Department for funding and preparation of check issuance.

Exclusions
- Suicide or self-inflicted injury
- Drug use or alcohol intoxication-related incidents
- Criminal acts committed by the employee
- Pre-existing illnesses not work-related
- Termination of employer-employee relationship with the Company
- Natural death (unless proven work-related and approved for burial assistance)
Employee Responsibilities
All covered employees must:
- Report incidents and submit claims promptly
- Provide truthful and complete documentation
- Keep beneficiary information updated
- Cooperate with any required validation or investigation
Failure to comply may result in claim disqualification and/or disciplinary action.
Program Authority
Covered customers will be required to sign an acknowledgment form confirming that they have received and understood the policy and guidelines stated herein.
Annex A – Expanded Accident Benefits
The following Expanded Accident Benefits shall be a guideline should a Customer be involved in accidents in the execution of duties and responsibilities during their respective working hours/shift.
Permanent Disability Benefit

Accidental Injury Benefits
